Computer-Assisted Coding (CAC) is a perfect example. It has transformed coding by improving efficiency and helping coders navigate increasingly complex medical records. But when CAC suggestions are accepted without independent validation, that strength can quietly become a coder's Achilles' heel.
As an Internal Quality Audit Manager, I have seen how taking one extra moment to validate documentation can prevent many avoidable coding discrepancies.
More often than not, the difference between an accurate code and an audit opportunity is not coding knowledge—it is taking that extra moment to verify the documentation.
The issue is not the technology itself. The key is using CAC thoughtfully, allowing professional judgment and documentation review to guide the final decision.

Five Habits of High-Performing Coders
Validate the documentation—not the CAC suggestion.
Start with the medical record, not the highlighted text. CAC can surface potential codes, but the documentation must support the final assignment.
Read beyond the highlighted sentence.
History, assessments, operative notes, pathology results, diagnostic reports, and discharge documentation can all change the coding story. Reviewing the complete encounter helps ensure the selected code accurately reflects the patient's clinical picture.
Pause when something feels "too easy."
If a diagnosis seems clear after reading only one sentence, take a moment to verify it against the complete encounter before accepting the suggestion.
Know when to query.
Submitting a compliant query reflects professionalism—not uncertainty. When documentation is unclear, a query helps protect coding accuracy, compliance, and defensibility.
Remember who owns the final code assignment.
CAC suggests. The encoder assists. The auditor validates. Ultimately, the certified coder is responsible for the final code selection.
The Auditor's Perspective: Follow the Documentation Trail
From an audit perspective, the goal is not simply to identify discrepancies, but to support accurate, defensible coding by confirming that every reported code is clearly supported by the medical record.
One simple question guides every audit: "Where is the documentation that supports this code?"
When that support is easy to locate and aligns with coding guidelines, the code becomes easier to defend, explain, and confidently stand behind.
Auditors approach CAC suggestions by following the documentation trail (after reviewing the entire record) and then determining whether the suggested code accurately reflects the encounter.
