This edition of Start Smart comes in the form of an important announcement. I want to make you aware of a proposed change for 2027 involving G2211. As you know, G2211 is an add-on code for visit complexity that is inherent to E/M services when the provider is the continuing focal point for all needed care or for a single serious/complex condition. What You Need to Know
Add-on Code G2211 Proposed Changes for 2027
CMS has proposed significant changes to G2211 in the 2027 Medicare Physician Fee Schedule. If finalized, these changes would affect how G2211 is billed and reimbursed.
Proposed 2027 Changes
G2211 would no longer be billed as a separate HCPCS add-on code.
Instead, it would become a modifier appended directly to the office/outpatient E/M code.
Rather than receiving a flat reimbursement, payment would increase by 16% of the base E/M reimbursement, allowing payment to scale with the level of the visit.
Current G2211 Use

G2211 is intended for providers who serve as the ongoing focal point for a patient’s care or who manage a single serious or complex condition over time.

When G2211 Applies
Office/outpatient E/M visits (99202–99215), home/residence E/M visits, & certain Preventive Services
Ongoing management of chronic or complex conditions.
Visits that advance a documented longitudinal care plan through medication management, care coordination, or chronic disease management.
Documentation Review Tips for Coders & Auditors

When reviewing documentation for G2211, verify that the medical record supports the following:

The provider is the continuing focal point for the patient's overall care or for management of a serious or complex condition.
The encounter advances an ongoing, documented care plan, rather than addressing only an isolated or acute problem.
The note reflects longitudinal care, including chronic disease management, medication adjustments, care coordination, review of outside records, or shared decision-making.
The documentation demonstrates complexity beyond the chief complaint, such as management of multiple comorbidities, high-risk medications, or coordination with other clinicians.
Example documentation statements:
“I serve as the patient’s primary care clinician and ongoing focal point for management of their chronic and acute conditions.”
“Today’s visit reflects ongoing longitudinal care; I continue to coordinate and integrate care for this patient’s multiple chronic conditions across settings and specialties.”
“We reviewed interval events, test results, and outside records and updated the patient’s chronic care plan, including medication adjustments and follow-up arrangements.”
Bottom Line

While these billing and reimbursement changes are proposed for 2027 and are not yet final, they represent a significant shift in how CMS plans to recognize and reimburse longitudinal, relationship-based care. Stay informed as CMS releases the final 2027 Physician Fee Schedule.
