When venous circulation is severely compromised, the blood flow to the area becomes stagnant. In these situations, leeches have been used to restore blood circulation following reconstruction. The leech creates a puncture wound that will bleed for hours and prevent the blood from clotting. Its saliva contains a potent anticoagulant, hirudin, which relieves venous congestion by inhibiting the conversion of fibrinogen to fibrin. The leech saliva also acts as a local vasodilator and local anesthetic.
